Answer :

Not necessarily. Someone could have tried to log in as you from another machine and failed. Your machine could have lost its cookies if you cleaned them out or for a number of other reasons. You might have mistyped your password, and then it asks for two-step. I’m sure there are a number of other reasons that are innocent that could cause this. You have to rule out a lot of other reasons before getting to the paranoid reason.
