If you’re talking about a wallpaper then you go to photos, press the button on the bottom left, and press set as wallpaper. If you are talking about a full blown theme (sounds, background, wallpaper) then you have to have a jailbroken iPhone. If to do and you’ve downloaded the theme from cydia, then you download winterboard and press apply.

Last Answer : Found after googeling a bit (not mac user), maybe you can have some use of the info: drag and drop your JPEG, GIF or TIFF file into background box in the Settings Pane from the ... file back in the Background box within the Settings Pane - this will automatically knock out the original theme!
