How often do you go to your settings/history area on Chrome? If you go to History and select Clear Browsing Data, there is a list of checkboxes of different things that Chrome will clean, if you tell it to. If the Stored Passwords checkbox is checked, then it will delete yours. Also, if you are using utilities such as BleachBit or CCleaner, and the settings are picked, it will do the same thing,
