What is the rule for equipment with a lockout tag attached?

Answer :

A lockout tag should only be removed by the person who attached the tag in the first place. Anyone removing the tag could actually cause injury or death to an electrical/engineer working on machinery, an electrician/engineer believing that all is safely switched off. I would sack any person who decided to remove a lockout tag without authorisation.
