Answer :

It could be due to improper pruning.  Different types of hydrangea need different types of pruning because of where they set buds.  Not knowing what kind you have I can't tell you 100% if this is the cause, but it's most likely.  Determine the knd you have and then Google pruning directions for that kind of hydrangea.
