What is Basal Implantation?
Basal implantation is one of the most modern and least traumatic dental restoration technologies. This method is based on the implantation of specially designed implants into the deepest, basal layers of bone tissue, which stimulates natural physiological processes and promotes the best regeneration of atrophied areas of the bone. That is why, basal implantation makes it possible to quickly restore the chewing functions of the jaw, as well as restore the aesthetics of a smile in a short time and with a minimum of discomfort for the patient.
What is a Basal Implant?
It is a construction consisting of a titanium post with threads or basal plates at the base and with an abutment or thread for fixing it in the upper part. The uniqueness of this dental technology lies in the fact that prosthetics are possible even in difficult clinical cases with the achievement of the necessary stabilization of prostheses in the bone.
Installation of implants
The procedure is carried out by maxillofacial surgeons and surgeons, implantologists, it is performed using local anesthesia, on an outpatient basis. Modern means of anesthesia used in this operation completely exclude the possibility of painful sensations and discomfort, the maximum that the patient feels is a slight vibration when preparing the bone for the installation of the post. The time allotted for the operation depends on 2 factors: the number of implanted implants and the place of their installation. On average, the operation takes 1 to 4 hours.
Sequence of procedures for basal implantation
Patient preparation - examination, anesthesia.
Formation of the prosthesis model. The features of the prosthesis depend on the structure of the jaw, the thickness of its bone tissue and the location of the implanted structure. The main condition for modeling is a uniform distribution of the load on the prosthesis when chewing.
Removal of diseased and loose teeth. Already at this stage, a pin can be installed in the hole formed after the extraction of the tooth.
Implementation and installation of the implant.
Fixation at the desired angle of the abutments.
Making an impression for a prosthesis.
After 3 days, the metal-plastic prosthesis is attached to the abutment.
In cases of subsidence of the gums or significant atrophy (1–2 mm) of the bone tissue, the prosthesis can be installed with an artificial gum for the external aesthetics of the dentition. The initial prosthesis made of metal-plastic is installed for splinting (strengthening, engraftment) of implants, while being a relatively light structure, it allows them not to be overloaded at first and, if necessary, makes it possible to correct the prosthesis according to the bite. Six months later, if desired, the metal-plastic prosthesis can be changed to another.
Indications
absence of 3 or more teeth;
loss of all teeth (edentulous);
lack of the possibility (medical indications) of bone augmentation;
periodontitis;
allergy to traditional removable dentures;
not wide alveolar ridge;
single absence of teeth.
Contraindications
Despite the versatility of this technique, there are a number of contraindications for which dentists do not recommend basal implantation.
Absolute contraindications:
treatment with drugs of the bisphosphate group;
diseases of the hematopoietic organs;
pregnancy and lactation;
diseases of the nervous system (central, peripheral);
oncological diseases.
It is possible to postpone the basal implantation procedure in the following cases:
exacerbation of chronic diseases (rheumatoid arthritis, osteochondrosis, hypertension, etc.);
neuroses, temporary depressive conditions and mental disorders;
the presence of swelling and inflammation in the oral cavity.
