How to tactfully refuse to attend a birthday party?

My brother and I were invited by my brother to celebrate his jubilee birthday, but due to the distance (350 km), the woman's long-term incapacity for work and my aversion to my sister-in-law, I do not want to be a participant in the celebration. How to get away with it? PS I have a gift ready.

It depends on your relationship. Sometimes the truth, no matter how cruel, is better than a tactful lie. Caring for a sick wife is reason enough. The distance even for a new pensioner and grandfather is a sufficient reason. Aversion and intolerance, the theoretical possibility of an unpleasant situation, is a reason like thunder.
