I guess a person can give a young girl this title if she is very shy and beautiful You'd basically need to really understand the Japanese culture and why they would give a shy girl such a title. There is no disrespect behind this and I think it is just a way to describe a shy person who is beautiful and has no way of knowing this is true. Sometimes people are so shy they are afraid to speak out or be noticed.
