Answer :

I have actually done that many times, but usually the words are similar words – either in the way they look or in meaning. ‘Entirely’ and ‘with’ don’t seem to fall into that category. Interesting. When it’s happened to me, I attribute it to a couple of factors: Hands working faster than brain; tiredness; or tipsy.
